1.I understand that my child(ren) may be participating in physical activity, such as Games and/or outdoor play, and with that there are risks for injury. I fully accept this risk and do not hold Hanson Baptist Church or any person involved in the Ministry Liable.

2.In the event of an emergency that requires medical treatment, every effort will be made to contact me or the emergency contact(s) listed. If those persons cannot be reached I give permission to the Hanson Baptist Church volunteers to seek treatment from a licensed physician for my child(ren)'s well being. I assume responsibility for all costs connected to any accident or treatment of my child(ren).

3.I grant permission for pictures of my child(ren) to be taken. These pictures may be used on our bulletin board, church directories, promotional videos, and church website.
